ASQ is a post-task questionnaire with three items. It measures overall ease of task completion, satisfaction with completion time, and satisfaction of support information.  It is measured on a 7 point scale (strongly agree to strongly disagree).

A questionnaire used immediately after usability testing to measure participant satisfaction and perceived usability of a product. Standardized usability questionnaires have questions follow a specific format and order and are designed for repeated use. These questionnaires have undergone psychometric qualification to measure its reliability, validity, and sensitivity.

Quantifying the User Experience: Practical Statistics for User Research

QUIS is a post-study questionnaire used to measure users' overall satisfaction and with specific aspects of the human-computer interface (screen factors, terminology and system feedback, learning factors, system capabilities, technical manuals, online tutorials, multimedia, teleconferencing, and software installation). You can pick and choose sections that are more applicable to the product you are testing. There is a licensing fee required. Each item is measured on a 9-point bipolar scale. There are two versions of the test: short (41 items) and long (122 items). 

QUIS (Questionnaire for User Interaction Satisfaction)

SUMI is a post-study questionnaire used to measure a product's general usability factor and five sub-factors. The sub-factors are efficiency (measure of how the software helps users complete their work), affect (general emotional reaction to the software), helpfulness (how much of the software is self explanatory and if there are documentation and help users can access), control (is the user in control of the product), and learnability (the speed in which users master the system and learn new features). The questionnaire involves a total of 75 items and requires a licensing fee. Each item is a statement (mixture of positive and negative) that can be answered by a three step scale (agree, undecided, disagree).

SUMI (Software Usability Measurement Inventory)

PSSUQ is a questionnaire used to measure users' satisfaction with a computer system or application. Four areas are measured: overall, system quality, information quality, and interface quaity. The newest third version of the questionnaire includes 16 items. Statements are scored on a scale from 1-7 (strongly agree to strongly disagree). The PSSUQ does not require licensing fee.  

PSSUQ (Post-study System Usability Questionnaire)

SUS is a post-study questionnaire that measures a product's usability. It is a fast questionnaire with 10 items with 5 scale steps (strongly disagree to strongly agree). SUS does not require licensing fee. This questionnaire should be taken right after the the session. The participant is required to answer all items without much introspection.

ASQ (After-scenario Questionnaire)

SEQ is a post-task questionnaire. There is only one item and it addresses the overall ease of completing a task. The practitioner has the option to use a 5-point or 7-point scale. 

SEQ (Single Ease Question)

SMEQ is a post-task questionnaire with a single item. The rating scale ranges from 0 to 150 with nine labels from "Not at all hard to do" to "Tremendously hard to do". The participants mark their answer by drawing a line through the scale (physical questionnaire) or using a slider to adjust their answer (online questionnaire). 

SMEQ (Subjective Mental Effort Question)

ER is a pre- and post-task questionnaire. The participant will answer expected difficulty of the task before starting on the task. Afterwards, they will rate the perceived difficulty of the task. 5-point or 7-point scales can be used.
